# Failure postmortem

A prior agent run failed. Produce a blameless postmortem from evidence.

## Inputs
Prefer `session-tape` JSONL or pasted tool logs. If missing, say what evidence you need.

## Sections
1. **Timeline** — what happened in order
2. **Impact** — user-visible + spend burned
3. **Root cause** — one primary cause
4. **Contributing factors** — secondary
5. **Fix** — concrete code/process change
6. **Prevention** — checklist item to add to mission briefs or tool discipline
7. **Follow-ups** — owners/actions (or “operator”)

Do not speculate beyond the tape. If the failure was cost overrun, cite the cost governor policy.
